Concussion is one of the most common and most underestimated injuries following a car accident, fall, or sports impact. Many patients are told at the ER their scan looked normal and sent home without a concussion diagnosis, without a treatment plan, and without documentation. Weeks later they are still suffering — and wondering why they are not getting better.

A concussion is a mild traumatic brain injury caused by a biomechanical force — direct or indirect — to the head or body. You do not need to strike your head directly. The whiplash mechanism of a rear-end collision is sufficient to cause concussion-level neurological disruption.
Concussion symptoms fall into four main categories:
Emergency rooms use CT scans to identify bleeding, fractures, and structural brain damage — not concussion. When an ER provider tells you your scan is ‘normal,’ they are telling you there is no bleed or fracture. They are not telling you there is no concussion. Without a clinical concussion assessment using standardized tools, concussion simply cannot be ruled out or documented.
Symptom Assessment — Validated symptom rating tools quantify your concussion burden across all four symptom domains, creating a baseline tracked across visits.
Neurological Examination — Complete neurological assessment including cranial nerve function, cognitive screening, and motor and sensory testing.
Balance and Vestibular Testing — Concussion disrupts the vestibular system. We assess balance function to identify vestibular involvement and guide treatment.
Oculomotor Evaluation — Eye movement abnormalities are among the most reliable clinical indicators of concussion. Our evaluation assesses tracking, convergence, and saccadic eye movements.
Concussion after a car accident almost always co-occurs with cervical spine injury. We evaluate and treat both simultaneously.
When symptoms persist beyond the expected three to four week recovery window, the condition is classified as post-concussion syndrome. CAIC manages PCS with a structured program combining vestibular rehabilitation, cervical spine treatment, and symptom-guided progressive activity.
Concussion in younger patients carries additional considerations that differ meaningfully from adult presentations. The developing brain is more vulnerable to the effects of traumatic disruption, and recovery timelines in children and adolescents are typically longer than in adults. School performance, social behavior, sleep quality, and emotional regulation are all frequently affected — and teachers and parents may not immediately connect these changes to a recent accident or fall. CAIC evaluates pediatric and adolescent concussion patients using age-appropriate protocols, provides return-to-learn documentation alongside return-to-play clearance, and coordinates with parents throughout the recovery process.
Both too much rest and too early a return to exertion are harmful. One of the most important decisions in concussion management is timing the return to physical and cognitive activity.

Concussion recovery is not a single-modality process. The cervical spine injury that almost always accompanies post-accident concussion needs chiropractic care. Concussion is the injury the ER is least equipped to diagnose. CT and MRI scans are normal in concussion patients by definition — the injury is functional, not structural. ER physicians conducting a 15-minute evaluation are not in a position to administer the SCAT5, conduct a King-Devick oculomotor assessment, or evaluate vestibular-ocular integration. That’s not a criticism of emergency medicine — it’s a description of what it is designed for. For concussion, CAIC is the appropriate next step after the ER.

Most adults recover in three to four weeks with appropriate management. Recovery is slower when concussions are not identified promptly, when patients return to full activity too quickly, or when cervical spine injury is not treated concurrently.
